E-Mentee Contract

I agree to the following Center for Innovation E-Mentorship expectations:

Before the beginning of the E-Mentorship:

       Step 1: Complete the Mentee or Mentor Application.

       Step 2: Mentee needs to prepare a one-page rationale as to why you are        requesting a mentor.  The one-page rationale should include the industry        you are interested in, and area(s) of expertise you are seeking help. Email        the one-page rationale to csciaca@mscd.edu

       Step 3: Review and electronically sign the Contract, submit and use the        back button to return to this page.

       Step 4: Review the handbook

       Step 5: The Center for Innovation will contact you via email once the        application, one-page rationale and contract is completed, with a        temporary username and password.

During the E-Mentorship

  1. Communicate with the E-Mentor weekly.
  1. Discuss accomplishments, areas of refinement and other topics of interest with your E-Mentor.
  1. Share and discuss ideas and practices with fellow mentee’s in the forum.
  1. Complete and return all Center for Innovation E-Mentorship surveys by their due dates.
  1. Notify the Center for Innovation in any changes to your student status during your mentorship commitment period.

 

I agree to the following Center for Innovation E-Mentorship policies:
Professionalism:
I am agreeing to maintain professional standards in all of my contacts both at my school site and in the E-Mentorship forums.

E-Mentorship Netiquette:
I am agreeing to refrain from the use of inappropriate language that is obscene or offensive. Center for Innovation reserves the right to remove any posting that is off-subject, obscene, offensive or deemed inappropriate by the Center for Innovation Lead E-Mentor including inappropriate or sensitive materials. Illegal activity such as harassment, stalking, or assault is prohibited and will be reported to appropriate law enforcement authorities.

Confidentiality:
I am agreeing that no confidential information be shared in E-Forums including posting any identifiable information such as names, addresses, phone numbers, and personal identification numbers.  Matters of a personal or sensitive nature should be discussed outside the E-Forum with the Mentorship Coordinator/Lead E-Mentor via private email or telephone.

Privacy:
I am agreeing that Center for Innovation E-Mentorship Program participation is password-protected and that only the E-Mentees approved by Metropolitan State College of Denver have access to its contents.  I understand that my participation in public E-Forums is accessible to all participants and that while password protected, that no online medium can be completely secure.

Participant Opinions:
I understand that the E-Mentorship and the contents of E-Forum result in the expression of many points of view on the same issue.  Participants must understand that the opinions expressed by individuals through email and on the E-forum do not necessarily represent those of metropolitan State College of Denver or the Center for Innovation.

Personal Responsibility:
I understand that Center for Innovation participants must take responsibility for what is posted to the E-Forums and that they maintain confidentiality and protect the privacy of other Center for Innovation participants.  E-Forum messages may not be shared or used as documentation for any purpose/ reason.

Center for Innovation Research:
I understand that E-Forum messages may be used by the Center for Innovation for research purposes.  Identity will be changed to protect the privacy of participants. Usernames and passwords are unique and may not be shared. Sharing usernames and passwords directly violates the privacy of Project REACH participants.  Non-compliance may result in restricted access to E-Forums.

Agreement:
As a Center for Innovation E-Mentorship recipient, I agree to the conditions of the program policies and procedures as stated above. Enforcement of the policy is the responsibility of the Center for Innovation Mentorship Coordinator/Lead E-Mentor.  (A typed electronic signature is the same as a hand-written signature.)

If you are student under the age of 18, parental consent must be received in writing before you can participate in the E-Mentorship program.
