Innovation Loan Fund Application Checklist

For all applications:

Completed application (use additional pages for monthly income and expense information if necessary.)

Loan application fees (non-refundable): $25

Proof of required business development courses (3 completed classes from either RMMFI, Metro State Center for Innovation, or others as approved.)

Business Plan (Financial, Operations, Marketing, Administration) for your business along with a detailed explanation of use of funds.

Copy of two most recent paystubs (no more than 90 days old.)

Copy of 2 most recent years’ personal tax returns (if you need copies of your tax return transcript please call 1-800-829-1040 and ask for the Individual Accounts department.)

12 month financial projections.

Existing businesses please also include:

Last 2 year’s Profit and Loss Statement and most recent available quarterly statement.

Last 2 year’s Balance Sheet and most recent available quarterly statement.

Copies of the business’ tax returns for the 2 most recent years.
